Introduction

The purpose of this document is to request access to official documents under the Freedom of Information Act, 1999, pursuant to Section 13.

The Request for Access to Official Document(s) form must be completely filled out and mailed to:

  • The making if this request is free of charge, except where the document requested is to be given in the form of a copy, printed or otherwise, in which case the applicant will be required to pay the prescribed fee.
