REQUEST FOR INFORMATION
Where, under a contract of insurance, a person is to receive an amount by way of an indemnity payment the VAT office must state whether or not the insured person is registered for VAT.
Procedure

Application Form (VAT 604)

Complete Vat 604 form (all questions must be answered)

Submit completed Form to counter clerk

VAT Office issues Confirmation of Vat Registration(VAT 605)
